Appearance Request

Requester Requirements
- Appearances must be booked one month in advance of the actual appearance date.
- Requestor will be sent an invoice for payment after the event is completed. Payment instructions will be included with invoice.
- The Spirit, Pride, & Tradition Program is accepting appearance requests for personal engagements on a limited basis. (i.e. weddings, birthday parties...)
- It must be understood that the maximum appearance time is 2 hours.

- Due to academic and time restrictions, appearance requests that fall during scheduled academic calendar breaks (i.e. spring, summer, holiday break) will be made on a limited basis.
- Appearance requests are more likely to be fulfilled if they fall outside of the academic day.
- Appearances will not be made on home football Saturdays and there will be limited availability the Friday prior to a home football game.
- Jonathan may not make appearances in bars.

*Please note* Request more than a 15 minute radius of campus require either transportation or payment of the standard IRS Mileage Rate.
Cancellation Policy
We reserve the right to cancel an appearance due to illness, personal emergency, academic issues, etc. If the appearance is cancelled by us, you will receive a refund. If the appearance is cancelled by the Requester, payment for services may still be due..
You will receive an email when your form has been submitted of your request details for your records.
You will receive a separate email from the SPT program regarding the status of your request and any other information that may be needed.
Appearances are generally scheduled 2-4 weeks in advance.

If your request is approved, you will receive an invoice after the event.
